Some people have asked for additional information, so below are some actions you can take pro-actively, should you wish to become a tissue & organ donor.
#1.get an orange sticker for the front of your driver's license
#2.sign and date the back of your driver's license
#3.asend an email to Wis DOT at: driverrecords.dmv@dot.state.wi.us with a statement stating your wishes along the lines of "I would like my record to reflect that I would like to be an organ and tissue donor". Make sure to include your name, current street address, city, state, zip, and your Wis DL#
OR
#3.bgo to www.donatelifewisconsin.com and select "Become A Donor". Fill out the requested information, including the statement of intent
#4.
complete the attached Organ & Tissue Donation form and share it with family.
The important news that come out recently is that the Organ & Tissue donation form will become the basis for an First Person Consent registry that is scheduled to become available online mid-2009. In the meantime, some of the steps listed above should help insure that your wishes are carried out.
